Adult License Waiver Program This two (2) part program is for Adults (ages 18 and over) that have a valid Learners Permit (issued by the Virginia DMV) who are looking to WAIVE the DMV required 60 days holding period AND also BYPASS taking the final road test at a DMV location! This program is NOT for brand new drivers who have never driven before! ADULT MEMBER RELEASE AND WAIVER OF LIABILITY PLEASE READ THIS DOCUMENT CAREFULLY! THIS IS A LEGAL DOCUMENT THAT (I) INCLUDES AN ASSUMPTION OF RISK, A WAIVER AND RELEASE OF LIABILITY, AND A COVENANT NOT TO SUE, AND (II) AFFECTS YOUR LEGAL RIGHTS! ADULT MEMBER RELEASE AND WAIVER OF LIABILITY.
Adult Participant Release & Waiver of Liability and Indemnity Agreement PLEASE READ CARFULLY. THIS DOCUMENT AFFECTS YOUR LEGAL RIGHTS AND IS LEGALLY BINDING. BY SIGNING THIS AGREEMENT, YOU ARE RELEASING YMCA OF GREATER OKLAHOMA CITY FROM ALL LIABILITY AND FOREVER GIVING UP ANY CLAIMS THEREFOR Assumption of Risk. Adult Supports Waiver – The Adult Supports Waiver provides services to individuals who meet the eligibility criteria and require at least one home- and community-based waiver service per month. Participants do not require the levels of support provided under either the Community Living Waiver or the Intensive Supports Waiver.
GENERAL RELEASE I acknowledge and agree that this Agreement covers and is intended to release and provide other benefits, legal protections and consideration to KAM Sharp Enterprises Inc., and its respective and collective agents, owners, officers, managers, directors, shareholders, affiliates, volunteers, participants, employees, and all other persons or entities acting in any capacity on. The Medically Fragile Adult Waiver offers services to individuals who have serious and complex medical needs and are no longer eligible for services under the Healthy Children and Youth program. To be eligible, an individual must be age 21 or older and assessed as requiring placement in an ICF/MR absent these services.
Adult Supports Waiver The Adult Supports Waiver provides services to individuals who meet the eligibility criteria and require at least one home- and community-based waiver service per month. Participants do not require the levels of support provided under either the Community Living Waiver or the Intensive Supports Waiver. The Aged and Disabled (A&D) Waiver allows individuals who are aged, blind, or disabled to remain in their home as an alternative to nursing facility placement.
The Ohio Home Care Waiver allows individuals with physical disabilities and unstable medical conditions to receive care in their homes and communities instead of nursing facilities, hospitals or rehabilitation facilities. It also allows these individuals to have more control of their lives and remain active participants in their communities.
